Social Prescribing

The practice now pleased to announce that we are able to provide an additional resource to help patients who may find themselves in the situation whereby they require support. Below is a link for patient to self refer if they feel they could benefit from this service.

Please note the following criteria before completing the referral form:

Over 18’s –

  • Assistance with finances, debt, benefits, employment and higher education
  • Low grade mental health issues eg sleep issues, anxiety, low mood.
  • Social isolation and keen to connect with others or community groups
  • Physical inactivity
  • Over/under weight and wanting support to tackle this
  • Issues following COVID eg isolation, bereavement, loss of income/employment, loss of support networks.
  • Family – parental separation, family illness, young carers, bereavement, children witnessing domestic abuse/drug + alcohol abuse
  • Employment, financial, higher education, careers advice
  • Weight management
  • Smoking, Alcohol, Drug use
  • Relationship advice – sexual health, relationship breakdown
  • Homeless support
  • Refugees/asylum seekers
  • Learning difficulties
